What You Need to Know About Your Appointment
Whether you are familiar with chiropractic care or this is your first time, you can expect your first visit to Natural Life Chiropractic to be different from your typical healthcare experience. Your initial paperwork will help our team get a better idea of your health history and current needs, and can be completed online and brought with you or filled out in our office prior to your appointment. During your first visit, our doctors will explain to you chiropractic and its benefits, the different services our office provides, and which services may be the best fit for you in reaching your health goals.
Before treatment, our doctors will ask questions about you and your family’s health history as well as your health goals and expectations. They will then complete a thorough physical examination and perform x-rays (if needed) to determine if chiropractic is right for you. A typical new patient appointment will last approximately one hour, with subsequent visits lasting from 15-60 minutes depending on your treatment plan. If you are new to chiropractic you may feel anxious or nervous, so feel free to ask questions at your appointment. Our doctors are here to help in every way possible to make your chiropractic visit positive and productive!

Once downloaded, fill it out on your computer and print it, or print it and fill it out at home.
If you have filled out your paperwork ahead of time, please bring it with you along with your driver’s license, health insurance card, and a current list of medications you are taking, if applicable. Please arrive 15-20 minutes before your scheduled appointment time to fill out our paperwork in office if you did not fill it out online. You will remain clothed for your examination and treatment, so loose-fitting or non-restrictive, comfortable clothing is best as you will be asked to move around for range of motion assessments, as well as move in different positions on your belly, back and side on the adjusting table. Treatment on the first day typically consists of a chiropractic adjustment and spinal traction on our mechanical tables. Further therapies, such as electrical modalities, soft tissue therapy, or rehabilitation may be rendered depending on your body’s needs.
